MBA Academic Forms
Students should complete the Concentration Declaration Form to declare or drop concentration(s).
Course Waiver Request
Students who would like to request a core course waiver must review the criteria and complete a Course Waiver Request Form.
Grade Grievance Form
Any student with a grade grievance shall follow the steps in the Flowchart of the GWSB Process to Resolve a Grade Grievance.
GW Course Request Form
Students who have completed graduate-level coursework as a certificate degree seeking student at GW prior to official admission to the MBA program may request the coursework to be applied towards their MBA degree requirements by completing the GW Course Request Form.
Independent Study Request
Students interested in arranging an individualized academic project with a GW faculty member must submit the Independent Study Request Form.
Leave of Absence Request
Students who find it necessary to interrupt active pursuit of the degree must complete a Leave of Absence Request Form.
Leave of Absence Request for Active Duty Military
Students who are called up for active duty and find it necessary to interrupt active pursuit of the degree must submit a Leave of Absence Request for Active Duty Military Form.
Non-Degree Course Request
Students who have completed graduate-level non-degree coursework at GW prior to official admission to the MBA program may request the coursework be applied towards their MBA degree requirements by completing the Non-Degree Course Request Form.
Petition for Elective Credit
Students who wish to satisfy a MBA concentration elective or general elective requirement with a course that has not been pre-approved must submit a Petition for Elective Credit Form.
Policy Exception Request
Students seeking an exception to a University or School of Business policy due to extenuating circumstances must submit a Policy Exception Request Form.
Program Transfer Request
Current students in any of the MBA programs at GWSB seeking to transfer into either the Professional MBA program or the Online MBA program must submit a Program Transfer Request Form.
Registration Transaction Form
Students who wish to request changes to their course schedules after the registration deadline has passed must submit a Registration Transaction Form to their academic advisor.
Students who would like to request reinstatement to the MBA program due to a break in enrollment or due to academic suspension must complete the Reinstatement Request Form.
Transfer Credit Request
Students who would like to request credits from another institution be applied towards their MBA elective requirements must complete a Transfer Credit Approval Form.
